Acupuncture for Bi Syndrome-
From S.G. a swollen knee
I started with a swollen knee problem back in 1992. A friend of mine
told me about the clinic; ever since then I've gone to the clinic
for all my aches and aliments.
Acupuncturist Wu is the best Chinese Medicine practitioner and cures
everything I discuss with him. I have a cold and now he is treating
me for that. I always carry around his cards with me and when people
tell me about their aches and pains, I tell them about Mr. Wu and
hand them his card. It is a blessing to have him, so I am just
spreading the good fortune I've found.
In western medicine, sciatica, rheumatic
arthritis, rheumatism, Rheumatoid arthritis, osteo-arthritis and
systemic lupus erythematosus all belong to something called the Bi
syndrome in Chinese Medicine.
Bi Syndrome is mainly manifested by groups of symptoms: pain,
soreness, numbness, swellings, heaviness, tenderness, inflammation,
burning sensation and deformation of the muscle, joints and/or
tendons, as well as restricted movements, such as in bending or
extending. It is due to the blockage of Qi and blood in the channels
and collaterals by invasion of pathogenic factors, such as wind,
coldness, dampness, and heat.
Bi syndrome is due to deficiency of vital Qi, the defensive Qi fails
to protect the body fighting diseases, therefore the pathogenic
factors have a chance to attack the meridians and collaterals.
Once the pathogenic wind, cold damp and heat invade the limbs, it
leads to blockage of Qi and blood, obstruction of channels and
collaterals, stagnation of phlegm and blood stasis. All of these
result in arthralgia -Bi syndrome.
Bi syndrome was first mentioned in the ancient Chinese
medicine book, Huang Di Nei Jing. This book divided Bi
syndrome into four major types:
Bi of five Zang
Bi of Six Fu
Bi of extraordinary organs
Bi of the limb
Here we only discuss the Bi of the limbs.
Because the symptoms of Bi of the limbs syndrome is related to some
modern diseases such as osteo-arthritis, rheumatoid arthritis,
Rheumatic fever, rheumatism, and spondylosis.
Etiology and pathogenesis:
1. Pathogenic wind, cold, heat and dampness invading the external
Due to living in a damp dwelling environment, wading in water or
getting caught in a rain or extreme sudden weather change, the body
becomes deficient in Qi. Therefore the wind, cold, dampness, and
heat sneak into the meridians and collaterals and stay in the
joints. Qi and blood flow are blocked. The Bi syndrome follows.
2.Transformation of deficient Heat:
Due to yin and essence being impaired by pathogenic factors for
period of time, the pathogenic factors are transformed into
deficient heat. The deficiency of heat impairs the production of Qi
and blood. Therefore the joints of limbs lose their nourishment and
circulation. Pain, swelling and inflammation follows.
3. Side effect of medicine intake
Due to long term consumption ofmedicine for expelling wind and
dry dampness. Or a person takes too much medicine to disperse
coldness or to clear heat. All of these factors can cause damage to
Qi and blood, yin and yang.
a. Phlegm retention and blood stasis:
Due to the pathogenic factors prolonged and aggravated by heat , the
Qi of channels and collaterals get blocked, thus the disorderly
movement of circulation would show, such as blood stasis or
stagnation of phlegm, obstruction of flows in channels and
collaterals. The stubborn Bi syndrome will then occur.
b. Deficiency of Qi and blood
After pathogenic factors invade the body and stayed in it, then the
blockage would become more persistent. The circulation of Qi and
blood will be easy to get obstructed and damaged. When the muscles,
bones or joints could not get enough nourishment from Qi and blood,
then the pain of the joints show up.
Insufficient yang caused by prolonged Bi syndrome results in the
vital Qi failing to protect the body and fight diseases. Pathogenic
invasions becomes more severe and nourishment from Qi and blood gets
After prolonged condition of yang deficiency with subsequent Qi and
blood deficiency, the deficiency of kidney and liver will follow
suit. This is the worst condition, because after the damage causes
subsequent yin deficiency, then the blood and Qi become even more
deficient, and the body nourishment and circulation would be even
less, leading to failure of the internal organs.
Diagnosis of Differentiation of syndrome
Clinically according to the characteristics of pain, Bi syndrome is
classified into four categories: Wandering Bi, Painful Bi, Fixed Bi
and Heat Bi.

the main symptoms of wandering Bi are unfixed pain, wandering pain
in joints and limbs.

relieved during the daytime and worse at night. Acute attack,
swollen joints with inflammation, redness on the inflamed area with
heat sensation. One prefers warmth, has a pale complexion with
reddish cheeks, red tongue with thin white coating, floating tight
or deep tight pause.
Mainly caused by pathogenic cold. The characteristic of
pathogenic cold is downwards movement, accumulation and contraction.
Therefore the painful bi is deeper and stabbing.
Stabbing and extreme fixed pain in the joints and limbs. Aggravated
by cold, relieved by warmth and pressure. Limited movements such as
bending and extending. Coldness sensation associated with pain.White
tongue coating. Taut and tense or deep, soft and taut pulse.
Mainly cause by pathogenic dampness. The pain characteristic is
heaviness and stagnation.
Fixed heavy and distending pain of the joints and limbs, swollen
joints. No redness in the pain area. Difficult to bend or extend.
Aggravated by high humidity climate. White greasy coating, with soft
and slow pulse.
4. Heat Bi
Caused by pathogenic heat or internal deficiency heat.
The main characters of pain are hot and burning.
Severe burning, swelling and excruciating pain.Inflammation with
redness in the affected area, accompanied with fever, thirst,

during daytime, worse at night.
5. Wang Bi
Chronic condition ofBi syndrome and causes Liver and Kidney
deficiency. Phlegm blockage, blood stasis and joint deformation
